Inspira Health has offered to assist with the COVID vaccination of students and staff at Delsea High School. COVID Vaccinations will be available on-site at the Delsea High School on Monday, May 24.
Those interested in obtaining the vaccine must register in advance and complete either the electronic consent embedded in the registration link below (for those 18 years of age and older) or the attached PDF consent for those under the age of 18 years. The paper consent must be completed and brought to the vaccination appointment. Additionally, you will find the Emergency Use Authorization (EUA) Patient Fact Sheet which must be provided to all parents who are giving consent to vaccinate minors. We will also provide a paper hard copy of the EUA fact sheet on-site to all those who are vaccinated.

All information collected in the registration process is captured securely and communicated directly to Inspira Health. Once registered, representatives from the high school will coordinate the scheduling of students and staff for vaccination on Monday, May 24. Please register using the link below:

For students that are 18 or older as of Monday, May 24, the consent form is embedded in the registration link above, and no additional paperwork will be required.
For students that are 16 or 17 as of Monday, May 24 a paper copy of the consent form that is attached to this email must be completed and brought to school on Monday, May 24.
All students are asked to bring a copy of their health insurance card, but please note that students will be able to receive the vaccine, regardless of health insurance.
Inspira Health is administering the Pfizer vaccine. Please review the attached Emergency Use Authorization Patient Fact Sheet for information specific to the Pfizer COVID-19 vaccine. Those electing to be vaccinated will be scheduled for their second dose 3 weeks after their first (June 14). Second doses will be scheduled when you arrive for your initial dose and will be conducted on-site at the High School. While the vaccination process itself takes minutes to complete, please expect to wait 15 minutes after your vaccination for your safety to monitor for any side effects. Those who have a history of allergic reactions for which they carry an EpiPen will be monitored for 30 minutes.
